Introduction
We are asking you to complete a survey about virtual care in your pulmonary rehabilitation (PR) program. Even if your program doesn't currently provide virtual care for PR, we still want to hear from you!
We want to know about virtual PR for a few reasons. We think that because of COVID-19, virtual care was delivered to people with chronic lung diseases across Canada at levels not seen before the pandemic. Changes to care delivery can have an impact on both patients and healthcare professionals! We want to learn about any changes to PR programs that may have occurred during the COVID-19 pandemic.
This survey has three parts:
- Part 1: PR program background
- Part 2: Current virtual PR
- Part 3: Future of virtual PR

Ethical Considerations
⚖ Benefits & Potential Risks of Participating in Survey
We cannot guarantee that you will receive any benefits from participating in this study. You may experience some satisfaction with helping us. However, the results of this study will be used to develop ways to improve delivering virtual programs for pulmonary rehabilitation across the country. No expenses will be reimbursed as part of this study. No honorarium will be provided for your participation in this study. There are minimal potential risks foreseen for study participants. Some questions asked in the survey may evoke sensitive issues, but you are free not to answer a specific question. Some confusion may arise from the questions, but you will be allowed skip any questions or follow up with the study team for clarification as needed.
🔏 Data Storage and Confidentiality
All information collected during this study will be stored for 7 years in a safe, secure and locked location at McMaster University. All electronic documents and data will be stored on a secure server without any electronic source identifiers. At the end of 7 years, all information collected during this study paper documents will be shredded, and electronic documents will be deleted. We will take care to protect your confidentiality. Your name and identifying personal information will not be used on any of the data collection forms. In addition to the researchers involved in this study, individuals from regulatory authorities or the Research Ethics Board may be granted access to the information collected during this study. These individuals are involved in monitoring and auditing research studies. You may request access to your data, which can include your survey responses and other information that you provided that we use in our final reports
🙋 Voluntary Participation
Your participation in this study is strictly voluntary. You can withdraw at any time before, during, or after the study. Should you choose to withdraw from the study at any point, all information from you will be destroyed. If you should decline to participate, it will not affect any future relationship with the investigators/facility and your data will be deleted.

This survey is part of a study that has been reviewed by the Hamilton Integrated Research Ethics Board (HiREB). The HiREB protocol number associated with this survey is [HIREB-14964]. The HIREB is responsible for ensuring that participants are informed of the risks associated with the research, and that participants are free to decide if participation is right for them.
