Office Access Request for Instructors to Record Courses

General Information

Phase 1 Access to Offices for recording course material does not signal a return of staff to campus for any reason other than to record quality courses that would otherwise not be possible in the home setting.  This form is to be used to indicate that you intend to use your office space on campus. Please note that not all buildings are available, and  your request may be denied.

Note: Access to your office does not guarantee that your office will be cleaned.  If you feel unwell especially on the day(s) you have permission to come to campus, please stay home.

Phase 1 Access to Offices for recording course material does not signal a return of staff to campus for any reason other than to record quality courses that would otherwise not be possible in the home setting.  No individual will come to campus to take part in activities that include (but are not limited to) group or individual meetings, literature review, paper writing, grant preparation, thesis defence, comprehensive exams, etc.

Briefly explain why you cannot continue to work remotely and require access to your office, including the names of the courses you intend to convert.
