Online Evidence-Informed Decision Making Skills Assessment Tool Feedback Survey

The National Collaborating Centre for Methods and Tools (NCCMT) has developed an online tool, the Evidence-Informed Decision Making (EIDM) Skills Assessment. This tool helps you to assess your current EIDM skills, provides tailored feedback and suggests resources from the NCCMT to address any identified gaps.

We are conducting a short survey to learn if you have used the Evidence-Informed Decision Making Skills Assessment, your experiences using the tool, and how we can improve the tool.

There are 12 questions in this survey.
This survey is anonymous.

The record of your survey responses does not contain any identifying information about you, unless a specific survey question explicitly asked for it.

If you used an identifying token to access this survey, please rest assured that this token will not be stored together with your responses. It is managed in a separate database and will only be updated to indicate whether you did (or did not) complete this survey. There is no way of matching identification tokens with survey responses.
